Our decision making is informed by our ethics, and in order to uphold these and the safety of ourselves, our family, our staff and our community (which includes you), we ask that you please join us in being responsible considerate humans with us & complete the information below prior to dining with us. By everyone agreeing to these terms, we can create a safe trusted space where you know the people around you are good humans too.

 

If a staff member or guest of Chambar test positive for COVID-19, contact tracing protocol will be followed by Vancouver Coastal Health.

Everyone who dines at Chambar is requested to sign this form to confirm that in the last 14 days you have not: 

 

a) tested positive for the virus; 

b) experienced symptoms associated with the virus; 

c) been outside of Canada; 

d) been in direct contact with someone who has been outside of Canada in the last two weeks, or someone who has tested positive for COVID-19.

 

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with dining at Chambar Restaurant.
